This form is for middle and high schoolers (6th–12th grade) who want to join us for Aiken Queer Youth (AQY) group meetings and events.

For Students:
This part is all about you! Go ahead and fill it out the best you can. If you get stuck, feel free to grab a parent or guardian to help you finish it.

For Guardians:
